General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Location: Golf Course Grounds: Employees were exposed to a defective sprinkler mechanism and valve assembly hazard. On or about 6/30/95, an employee was injured during the servicing of a Toro Model 690 Rotary Sprinkler when a stuck valve assembly unit shot out of the sprinkler like a missile and struck him on the face. Among other methods, one feasible and acceptable abatement is to ensure that all grounds keepers read the installation and service instructions manual for the Toro 690 Series Rotary Sprinkler and are thoroughly trained before they perform any service.
