employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck or crushed by moving parts of a
machine:
(a)  Location:  Jobsite;
On or about 11/13/95, an employee was fatally injured when rotating parts
from
inside
a Pioneer rock crusher pulled him inside the machine during the cleaning
of a discharge
chute.  The interior rotating parts pulled the employee inside while the
machine was still
in operation.
One feasible and acceptable abatement method among others to correct this
hazard is to
disconnect and lock out the power source before attempting to service,
clean or unjam
equipment as recommended by the manufacturer.
