Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of having part of their body caught between the moving material lift and the hoistway. (a) Location: At The Establishment; On or before 03/12/96, employees were exposed to crushing hazards from a material hoist that was not equipped with an appropriate interlocking doors and adequately maintained rope/hoist mechanism. Among other methods, feasible and acceptable abatement methods include, but not limited to, the following: 1. The installation of material lift car doors or gates that would prevent employees from having any part of their body outside the periphery of the lift, unless the lift is within the landing zone, and an electrical interlock on the lift to prevent the lift car from moving unless the lift car doors or gates are in closed position. 2. Periodic maintenance on the rope and winding drum mechanism to identify and repair potential problems.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling down the hoistway shaft when the hoist car was not in the landing zone. (a) Location: At The Establishment; On or before 03/12/96, employees were exposed to serious physical injuries since the material hoistway gate was not electrically interlocked and could be left open during the lift operation. Among other methods, one feasible and acceptable abatement method includes, but is not limited to: providing a lift hoistway, landing doors or gates that have electrical interlocks to prevent the lift from moving when the landing doors or gates are in open position and so latched that the landing doors or gates cannot be opened from the landing side unless the lift car is within the landing zone.ng
