Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls from ladders:
(a) Location: IRS Building, Andover;
On or about 8/9/96, employees were exposed to over six foot falls while
using defective
ladders.
Among other methods, feasible abatement methods include:
1.  All repairs to portable reinforced plastic ladders should be performed
by a
competent mechanic and meet or exceed the original design requirements
determined by the manufacturer.
2.  A periodic ladder inspection program should be developed and
implemented.
3.  Employees should be trained in inspection procedures to be followed
prior
to
use of any ladder.d
