Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
a.  Mill Department - On or about August 12, 1999, the elevator gates on
the first floor and
second floors did not completely enclose the elevator shaft openings when
the elevator was in
use.  Employees were thereby exposed to hazards associated with shaft
openings.
A Feasible method of correcting the hazard is to conform to the
specifications referenced in
"ANSI/ASME A17.1 Section 204.4/1993.
