(a)On October 9, 1996, at the Pick 'n Save store located at 15445 West
National
Avenue, workers painting the inside south wall at a height of
approximately 15
feet, from the platform of a Skyjack scissors lift (serial number 23849),
had not
secured the mid-rail chain at the entrance to the platform, prior to
elevating the
platform, as required by ANSI A92.6-1990, section 7.9(3).  This condition
may
have contributed to the accidental fall of a worker from the platform.
(b)On October 10, 1996, at the Pick 'n Save store located at 15445 West
National
Avenue, workers hanging plastic sheeting at a height of approximately 15
feet,
from the platform of a Skyjack scissors lift (serial number 23850),  had
not
secured the mid-rail chain at the entrance to the platform, as required by
ANSI
A92.6-1990, section 7.9(3).
