Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working in confined spaces were subjected to hazards, such as, lack of oxygen, fire and explosions: a.Edens separator, employees entered the Edens separator on November 9, 1994 and no provision was made to ensure the permitted conditions were maintained as referenced in ANSI Z117.1-1989, Section 5.3.2. Feasible means of abatement method(s), among others, include: Conducting continuous atmospheric monitoring (with alarm) for toxic and flammable vapors and oxygen deficiency. b.Edens separator, employees entered the Edens separator on November 9, 1994 and no provision was made to ensure the Edens separator was isolated and locked out as to prevent entry of flammable or combustible materials as referenced in ANSI Z117.1-1989, Sections 8.2 and 8.3. Feasible means of abatement method(s), among others, include: 1.Close and lockout the gate valve upstream of the Edens separator and divert the flow during employee entry. 2.Drain the Edens separator, close and lockout the gate valve upstream of the Edens separator and divert the flow away from the Edens separator during entry.
