A- Hwy.30, 3 miles off Hwy.21, Port Wentworth, Georgia, Crew 453 -
Dielectric tests were
not performed to the "Altec" truck mounted aerial lift S/N 1002CV1695 for
electrical insulating
components and systems, therefore exposing the operator and nearby
employees to electrical
shock hazards while using and moving this equipment as close as to 4 to 6
inches from the 7,620
volts overhead power lines.
Among other methods, one feasible and acceptable method to correct this
hazard is to develop
and implement an inspection schedule, adhere to the manufacturer's
operator's
manual and the
maintenance and parts manual, and to follow ANSI standard A92.2 to ensure
that this equipment
is always in good and safe working condition.e
