Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)1/4/95 at the jobsite: A freight elevator was in service which had
no front or rear door car enclosure gates, no top roof enclosure,
no internal lighting, no maintenance inspection certificate, no
interlocked safety systems at all or service test certificate as
required by ANSI/ASME A17.3-1968 and BOCA 1984.
Employees were exposed to serious fall and crushing injuries
should the elevator fall down the elevator shaft or employees fall
into the front or back wall or floor overhangs as the elevator
passes by.
Feasible and acceptable methods among others to correct the hazard is to:
(1)Comply with ANSA/ASME A17.3-1968 and BOCA 1984.
(2)Conduct periodic inspections of elevator as recommended by the
ANSI/ASME & BOCA Standards to insure integrity of interlocks,
ropes, & switches for gates & equipment & document above,
(3)Fully enclose the freight elevator car as recommended by the
standards.
(4)Provide internal illumination as recommended.
A search of the OSHA Standards has been conducted and there are no
applicable standards.
