Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls from aerial lifts:
Rush-Copley construction site, Economy Mechanical Industries, did not
ensure that aerial lifts
used on the site were used in accordance with ANSI A92.6.
Aerial lifts used by Economy Mechanical Industries did not have all chain
guards secured
while in use.
Feasible and acceptable abatement methods include but are not necessarily
limited to:
1.Ensure chain guardrails are used whenever aerial lifts are used.
2.Provide each platform with an appropriate operating/maintenance manual.
3.Provide daily inspection on the aerial lift for cracked welds, hydraulic
leaks,
damaged control cables, loose wire connections, exposed power transmission
gears, tire damage, and proper use of guardrails.
4.All modifications and alterations to work platforms shall be certified
in writing
by the manufacturer.
5.Provide operator training in the manufacturer's instruction and safety
rules.
