designed to prevent the exposure of an employee to the hazards of the band
building machines:
a. Dept. 412. Instructions addressing the squeegee sticking to the
building belts on the
band building machines were inadequate in the band building manual.
b. Dept. 412. Training of employees in operating the band building
machines did not
address the squeegee sticking to the building belt on the band building
machines.
c. Dept. 412. Instructions addressing the removal of squeegee wrapped
around the steel
bar between the building belt and the hugger belt of the band building
machine were
inadequate in the band building manual.
d. Dept. 412. Training of employees in operating the band building
machines did not
address the squeegee wrapping around the steel bar between the building
belt and the
hugger belt on the band building machines.
Among feasible abatement methods would be to:
1. Change the band building manual to address reversing the machine to
remove
squeegee sticking to the building belt.
2. Ensure that new learners are shown the proper procedure for removing
squeegee that
sticks to the building belt.
3. Change the band building manual to incorporate lockout procedures when
removing
squeegee wrapped around the steel bar.
4. Ensure that all current and future operators are shown the proper
lockout
procedure
for removing squeegee that is wrapped around the steel bar.
e. Dept. 430. Safety trip control on the cushion mill was not located to
stop the mill if
the operator got caught in the machine.
f. Dept. 412. Band building machine #8 had an exposed nip point.d
