Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falling from an aerial lift:
North Side of House. Swanson Plumbing did not ensure that aerial lifts
used on the site were
in accordance with American National Standards Institute (ANSI) Guideline
for Self-Propelled
Elevating Work Platform Standard, ANSI A92.6.
a)Aerial lift used by Swanson Plumbing had been placed on a one axle
trailer to increase
elevation.
b)Aerial lift used by Swanson Plumbing did not have its outriggers
extended.
c)A Ladder was placed on the aerial lift to increase the elevation to
reach the roof.
Feasible and acceptable abatement methods include but are not necessary
limited to:
1.Provide each platform with an appropriate operating/maintenance manual,
and
2.Use an aerial lift with enough extension to reach the work level or use
a ladder long
enough to reach the work level, and
3.Provide operator training in the manufacturer's instruction and safety
rules.
