climbing onto the
discharge area of the baler and using their feet to kick the boxes to
force them down into the
hopper or climbing into the hopper to move boxes by hand to clear jams.
a.  On or about January 19, 1996, the baler operator (janitor) climbed the
discharge area
of the baler, opened the access door to the hopper used his left foot to
push the boxes
down into the hopper.
b.  On or about December 12, 1995, the baler operator (janitor) climbed
the discharge
area of the baler, opened the access door to the hopper and jumped down
into the hopper
to clear a jam.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to provide effective training for employees servicing and clearing jams of
machinery
supplemented with procedures and those work rules necessary for safe
operation.
