Section 5(a) (1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment or a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling load due to failure of custom made lifting device: US Truck Body/Midwest: Custom made lifting device from angulars and bars welded together were used to lift manufactured truck bodies for assembly to the truck. Feasible and acceptable abatement methods include, but not limited to: Comply with ANSI B30.11 - Attaching the load on Underhung cranes and Monorail Systems and; 1.Provide rated load on any custom-made lifting device. 2.Provide initial, frequent and periodic inspection of the lifting devices. 3.Ensure that initial proof testing of 125of the rated load was conducted. CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.
