Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees Work Site - On or about 9/21/94, employees were exposed to the hazard of being struck by the Ford 9000 (1971) dump truck because it was not capable of sounding an alarm or similar warning device when operating in reverse and where the driver's vision to the rear of the vehicle was obstructed by the bed of the truck. Abatement Note: Among others, one feasible means of abatement would be to retrofit this vehicle with an alarm that sounds and is audible to the rear of the truck when the truck's reverse gear is engaged.
