causing or likely to cause death or serious physical harm to employees in

(a)Employees were lifted approximately 10 ft on a pallet by a Clark
forklift to reach
stored inventory and were not protected from falling.  The pallet did not
have:
a slip resistant floor, a restraining means (such as railings or personal
fall
protection devices), secure attachment of platform to forks or lifting
mechanism,
and protection from moving parts of the forklift.  A feasible means of
abatement
includes, but is not limited to, following ANSI/ITSDF B56.1 - 2009 Safety
Standard For Low Lift And High Lift Trucks.
Certification, with date and method of abatement required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.ment
