Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: A mechanical locking pin was not used in conjunction with the Hendrix Quick Coupler to ensure proper mating of the attachment to the coupler. (a)Link Belt backhoe equipped with a quick coupler was being used to perform excavation and sewer installation. A mechanical locking pin mechanism was not used to ensure proper matting of the bucket to the coupler contributing to the detachment and crushing injuries to a worker below the backhoe bucket. No certification required. Violation Abated.
