Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a) The employer did not furnish employment and a place of employment which
were free
from recognized hazards that were causing or likely to cause death or
serious physical
harm to employees in that employees were exposed to hazard of falls from
door openings
at heights up to 15 feet 4 inches on to concrete floors or equipment below
or being struck
by pallets and materials falling from 2nd floor or mezzanine door openings
or from
elevated loads during insertion or removal at door openings by inordinate
and inefficient
use of telescoping mast lift trucks.
