Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazard of falls from door openings at heights up to 15 feet 4 inches on to concrete floors or equipment below or being struck by pallets and materials falling from 2nd floor or mezzanine door openings or from elevated loads during insertion or removal at door openings by inordinate and inefficient use of telescoping mast lift trucks. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS...." 1. Install a material lift or 2. Use freight elevator Location: 1. Mezzanine work area (general store) and ground level (1st floor) warehouse A. 2. Second floor plastic production department west wall and ground level (1st floor) plastic warehouse on or about April 17, 1998.
