Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employee(s) were exposed to the recognized hazard of being crushed by a falling load and/or struck by a failed cable on or about August 27, 1998. Employee(s) were attempting to release a crane hook from a dam gate which was suspended from a cable system when a eye bolt attached to the cable system failed, causing the load to fall into the river below. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS...." 1. Have an engineering study conducted by a registered professional engineer on the method used to suspend the dam gates from the cable system, 2. Use a crane with the boom reach and lifting capacity sufficient to lift the dam gates into position without the use of the cable system.
