a) For the TMP Belt Manlift, 5th floor, on or about June 2, 1997,
employee(s) were subject to crushing or
falling hazards when employee(s) were unable to escape from the steps of
the manlift when the upper limit
switches fail or when the mechanical drive or power fail(s) and belt
reverses on the down run.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS..."
to install a fixed ladder accessible from both the up and down run of the
manlift for the entire travel as stipulated by
ANSI A90.1, 1969, RULE 107, as presently installed on this manlift for
other floors except the 5th.
In the Alternative:
29 CFR 1910.68(b)(12): The belt manlift was not provided a fixed metal
ladder accessible from both the "up" and
"down" run of the manlift for the entire travel of the manlift:
a) For the TMP belt manlift, 5th floor, on or about June 2, 1997,
employee(s) were subject to crushing or
falling hazards when employee(s) were unable to escape from the steps of
the manlift when the upper limit
switches fail or when mechanical drive or power fail(s) and belt reverses

ABATEMENT METHOD              TO CORRECT THIS HAZARD IS...."
to install an emergency escape grab-bar/rail and platform to permit the
rider to  swing free
should the emergency stops (switches) become inoperative as stipulated in
ANSI A90.1, 1969,
Rule 106(C) and as depicted in National Safety Council Data Sheet No.
1-4-1-81, Manlifts,
Fig.7.
In The Alternative:
29 CFR 1910.68(b)(11)(iii): For belt manlift(s), an emergency grab bar of
rail and platform was
not provided at the head pulley, when over 6 feet above the top landing,
to permit the rider to
swing free should the emergency stops become inoperative:
a) For TMP-belt Manlift, the head pulley and surrounding maintenance
platform were
16 feet above the top landing and a grab bar/rail and platform were not
provided
for the
rider's escape (swing free) in event of failure of the emergency stops
(limit switches).

29 CFR 1910.68(c)(5)(i)(a): For belt "manlifts", two separate automatic
stop devices were not
installed as required, in that, when both are split rail switches, the
second must be installed 6
inches above and on the side opposite the fist slit rail switch:
a) For the TMP Manlift, the split rail switches were improperly installed
on the same rail
or side.

Rule 401(a) and Operator's Manual, calls for
"hand pressure" test on the steps when reaching the limit switches.

physical harm to employees in that employees were exposed to:
a) For the TMP Manlift on or about June 2, 1997, employee(s) were exposed
to the hazard of being crushed
between the step and the overhead steel platform when the safeties or
limit switches were inoperable,
b) For the Millwright manlift, on or about June 2, 1997, employee(s) were
exposed to the hazard of falls and
crushing injuries in the event the limit switches or safeties were
inoperable,
c) For the TMP manlift, employee(s) were exposed to the hazard of falls or
other injuries in the event of failure
of safeguards provided by items such as but not limited to guardrails,
steps, step fastenings, belt and belt
tension, handholds and fastenings, lubication, warning signs and lights,

falls or other injuries in the event of
failure of safeguards provided by items such as but not limited to

for assuring operable safety stops/limit switches is to maintain a weekly
inspection log or record that will provide a
systematic work practice and help assure that the safety stops/limit
switches are checked/inspected weekly as
previously logged or recorded for the TMP and Millwright manlifts in March
3, 1994 and November 11, 1993,
respectively; for assuring safety provided by other items such as but not
limited to the mechanical, structural and other
safety items listed above restart the monthly inspection log last used in
December 1988.  In addition, both the weekly
and monthly inspection logs are required as stipulated in ANSI A90-1-1969
Rule 401(a).
In The Alternative:
29 CFR 1910.68(e)(3): Written records of findings were not kept for each
inspection of manlift(s):
Maintaning the required certification inspection record is a work practice
that helps insure that safeguards are in place:
a) For the TMP Manlift, the weekly inspection log for limit switches was
last recorded on March 3, 1994,
b) For the Millwright Manlift, the weekly inspection log for limit
switches was last recorded on November 11,
1993,
c) For the TMP Manlift, the monthly inspection log for the many
mechanical, electrical and other safety items
