General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of explosion or burns from unintentional release of pressure from steam jacketed, unfired pressure vessels that operate at 80-100 psi and 300 degrees F temperature, in that the quick opening door safety devices were inoperable. a) Safety devices for the #1 and #2 autoclaves had been bypassed in that the pressure activated door locking pins did not engage. The safety device system did not prevent admission of pressure into the autoclaves until the doors were fully closed and locked, nor were the doors prevented from being opened until pressure within the vessels was reduced to a safe level.
