General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe burns and/or death from the inadvertent admixture of: Polysolo DPM, a glycol ether and potassium permanganate. On or about 02/24/94, an employee manually added potassium permanganate to a dye kettle which contained Polysolo DPM. These chemicals in combination caused a violent reaction exposing the employees to severe burns. Among other methods, one feasible and acceptable methods to correct this hazard is to eliminate the use of potassium permanganate for use in water treatment or develop procedures to eliminate the possibility of intermixing these chemicals in the treatment process.
