recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being crushed by a forklift truck in the event of a
turnover:
(a)  Throughout the Shop; employees operating forklift trucks were not
required
to wear protective restraint devices (seatbelts) that were provided on the
Hyster
forklifts.
Among other methods, one feasible and acceptable method to correct this
hazard would be to
required employees to use available seatbelts and add this requirement to
the employee training
program.
