Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to inadvertent machine operation: (a) 430 Home Drive, Pittsburgh, Pa; The differential steering systems Twist Tiller Bar was sticking and the Twist Tiller Bar's indicator marks did not line up properly as a second home-made line had been etched into the plastic handle of the twist tiller bar. In addition, there were no readable numbers/lettering on the end of the Twist Tiller bar. All of these modifications were made without written approval by the manufacturer.
