Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries and/or death from falling loads and hoist parts: (a) Turret Steel Facility; Workers were operating the five ton rope operated overhead girder single hoists that were not inspected frequently or periodically, did not have preventative maintenance performed on the running ropes or hooks, did not have the rope control boards marked, nor were written reports prepared certifying inspections. Among one feasible and acceptable method to correct these items would be to inspect frequently, periodically, perform preventive maintenance conduct running rope and hook prepare written reports in accordance with American National Standards Institute - ANSI B30.16.
