Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to falling loads.
A) Service/ Repair Shop: The hydraulic automotive lift designated
as lift #1 located two repair bays North of the Service Office had
two safety catches that were rusted in the open position. In the
event of a hydraulic system failure, the load (typically an
automobile) would fall on the employee.
A feasible method to control this hazard is to free the catches of
corrosion, lubricate them and maintain them according to
manufacturer's specifications.

employees were exposed to exploding abrasive wheels.
A) At the North portal between the Service and Body Shops, the
Sears 3600 rpm, Model 19580 pedestal grinder showed evidence of
grinding on the side of the wheel. Employees were exposed to severe
injury in the event of the abrasive wheel exploding while grinding
on the side of the wheel.
B) At the South portal between the Service and Body Shops, the Blue
Point 1450 rpm, Model 696EL1128205 pedestal grinder showed evidence
of grinding on the side of the wheel. Employees were exposed to
severe injury in the event of the abrasive wheel exploding while
grinding on the side of the wheel.
A feasible method to control this hazard is to instruct employees
immediately to not grind on the side of the wheel and to explain to
the employees the potential for serious injury or death in the
event of an exploding wheel.
