Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that the
employees were exposed to severe multiple injuries or death from fall:
SERVICE/OUTSIDE AREA:  Employees were required to "climb up" the ladders
on the fire
engines/trucks while carrying tools to perform inspections, repairs and
maintenance on the
ladders including the "buckets" without wearing and/or utilizing
appropriate personal protective
equipment (PPE), such as (but not limited to) personal fall protection
systems and were exposed
to the hazard of fall any where from 10 feet to 110 feet to the ground.
Among other methods, one feasible and acceptable method to correct the
hazard is to:
1. Install "life lines" from the bottom to the top of the ladders and
utilize "rope grabs" along
with body harnesses.
2. Utilize different method(s) of performing the services.
3. Utilize other available and feasible engineering technologies.
