Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by overhead doors: In the following areas, electrically operated "Rapid Roll" doors used for the access of material handling equipment were in an unsafe condition where parts were defective and safety return switches were not operational: a) Building 2, South Airlock, adjacent to Final Assembly: Internal Door: Spring cover latch broken, safety return switch not operational External Door: Spring cover latch broken, safety return switch not operational b) Building 2, East Airlock, adjacent to Engine Wash: Internal Door: No safety return switch External Door: Spring cover latch missing, safety return switch not operational c) Building 2, East Airlock at Powder Room: Internal Door: Safety return switch not operational External Door: Spring cover latch broken, safety return switch not operational d) Building 2, North Door at Powder Room : Door does not operate. e) Building 2, East Airlock at Column M-21: Internal Door: Spring cover latch broken External Door: No safety return switch f) Building 3, North Airlock: Internal Door: Spring cover smashed, cover latch broken, safety return switch smashed External Door: No safety return switch g) Building 10, West Door: Safety return switch not operational h) Building 10, South Door: Safety return switch not operational, spring cover latch broken i) Building 15, South Door: Safety return switch not operational, spring cover latch broken, angle iron for external door control is loose Among other methods, one feasible and acceptable abatement to correct this hazard is to repair or replace defective hardware and perform a periodic maintenance check, ie quarterly or semiannually.
