Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of entering into and performing work within confined spaces: a) 26 Leigus Road, Wallingford: Employee(s) required to enter into and perform work within a manhole were exposed to an atmosphere that was immediately dangerous to life and health. On or about 3/15/95. Feasible abatement methods include but are limited to the following: 1. Develop and implement a confined space entry program which should include the following elements: a) Identification and evaluation of confined spaces. b) Develop and implement confined space entry procedures. c) Utilize state of the art direct reading instruments. d) Provide and ensure the use of appropriate protective equipment. e) Provide training and information to affected employees concerning the potential hazards associated with the confined space, proper use of personal protective equipment, communication between the entrant(s) and attendants(s), hazard recognition and emergency procedures. f) Develop emergency and rescue procedures.
