General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to severe multiple injuries or death fram fall: ESTABLISHMENT: The employees, required to perform maintenance and servicing tasks on the HVAC unit in the "pit" on the roof top and to change light bulbs in the lobby area were exposed to the hazard of fall from 10 to 30 feet to the lower levels as follows: 1. They were hand-carrying heavy objects, such as (but not limited to) a 6-foot stepladder and tools while ascending/descending the Werner non-self supporting portable extension ladder to access the flat roof over the vehicle driveway in the front entrance or vise versa. 2. They dropped the stepladder into the pit from the edge of a "small" high pitcehed roof (6 in 12 pitch) adjacent to the pit. 3. They entered into the pit from the edge of the pitched roof by stepping on top of the stepladder. 4. They were hand-carrying light bulbs and tools while ascending/descending the Werner non- self supporting portable extension ladder to access the ceiling over the floor partially containing carpeting. Among other methods, one feasbile and acceptable method to correct the hazard is to: 1. Prohibit the employees from hand-carrying any objects while using any types of ladders that can cause the employees to lose balance and fall. 2. Hoist up and down "objects" by utilizing appropriate hoisting equipment and methods to/from the roof, ceiling, and ground. 3. Utilize a properly designed, engineered and placed fixed or non-self supporting portable ladder on a wall of the pit. 4. Utilize other available and feasible technologies. FCLC Shelton LLC/AmeriSuites was previously cited for this violation of this Occupational Safety and Health Standard Section 5(a)(1), which was contained in OSHA Inspection Number 123162794, Citation 01, Item 001 which was issued on 07-19-2006, with respect to a workplace located at 695 Bridgeport Avenue, Shelton, CT, 06484.
