Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that employees
were exposed to the "hazards" associated with the overhead door failure:
ESTABLISHMENT/WASTE TREATMENT AREA:  The overhead door had the following
deficiencies:
1.  The spring on the right side of the manually operated overhead door
was broken.
2.  The Hyster powered industrial truck (PIT) was used to raise/open the
overhead door
for removal and placement of the sludge container.
3.  Two (2) pieces of 4-inch X 4-inch X 9-foot long wooden blocks were
placed on both
ends to support and keep the overhead door stay open.
Among other methods, one feasible and acceptable means to correct the
hazard is to:
1. Use the PITs only for the purpose for which they were designed; follow
the powered
industrial truck manufacturer's/owner's manual recommendations.
2. Replace or repair the overhead door.
3. Conduct frequent inspections and maintenance on the overhead door.
4. Utilize other engineering methods to have mechanical stop and opening
the door.
