recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that employees
were exposed to head/body crushing hazards and/or death:
WASH BAY:   The employer did not ensure through visual inspections,
functional tests and
conducting maintenance on a regular basis that all components and parts of
the power operated
overhead door (door) were kept in good and safe working conditions at all
times.
On/or about May 29, 2007, the cables of the door snapped during "closing"
cycle, causing the
door disengaged from the supporting guide rails and fell to the floor. An
employee was hit by
the door and sustained severe injuries from the incident.
Among other methods, one feasible and acceptable means to correct the
hazard is to:
1.  Perform periodic and regular inspections and maintenance on the door.
2.  Train employees in the recognition and avoidance of unsafe conditions
in conjunction with
the use of the door.
3.  Utilize other engineering methods.
