Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee(s) were exposed to severe multipe fractures, broken bones, and amputations: WORKSHOP: The overhead hoists, such as (but not limited to) the CM Lodestar and Handling Systems had the following deficiencies: 1. CM Lodestar overhead 1/2 ton hoist at the Lever cutter: a. Spreader bar load rating was not marked or coded. 2. CM Lodestar overhead 1 ton hoist at the Cameron rewinder: a. Spreader bar load rating was not marked or coded. 3. CM Lodestar overhead 1 ton hoist at the Aero paper slitter, entrance area: a. Spreader bar load rating was not marked or coded. b. Safety latches were left open by tapping onto hook throats/openings. 4. CM Lodestar overhead 1 ton hoist at the Aero paper slitter, exit area: a. Spreader bar load rating was not marked or coded. b. Safety latches were left open by tapping onto hook throats/openings. 5. CM Lodestar overhead 2 ton hoist at the REM paper slitter, entrance area: a. Safety latches were left open by tapping onto hook throats/openings. 6. CM Lodestar overhead 1 ton hoist at the Fales paper slitter: a. Spreader bar load rating was not marked or coded. 7. Handling System Coffing rail 1/4 ton, jib hoist 1/2 ton at the #15 spooler: a. Spreader bar load rating was not marked or coded. Among other methods, one feasible and acceptable method to correct the hazard is to: 1. CM Lodestar overhead 1/2 ton hoist at the Lever cutter: a. Mark or code the spreader bar with appropriate load rating. 2. CM Lodestar overhead 1 ton hoist at the Cameron rewinder: a. Mark or code the spreader bar with appropriate load rating. 3. CM Lodestar overhead 1 ton hoist at the Aero paper slitter, entrance area: a. Mark or code the spreader bar with appropriate load rating. b. Remove the tape and ensure proper functioning of safety latches. 4. CM Lodestar overhead 1 ton hoist at the Aero paper slitter, exit area: a. Mark or code the spreader bar with appropraite load rating. b. Remove the tape and ensure proper functioning of safety latches.5. CM Lodestar overhead 2 ton hoist at the REM paper slitter, entrance area: a. Remove the tape and ensure proper functioning of safety latches. 6. CM Lodestar overhead 1 ton hoist at the Fales paper slitter: a. Mark or code the spread bar with appropriate load rating. 7. Handling System Coffing rail 1/4 ton, jib hoist 1/2 ton at the #15 spooler: a. Mark or code the spreader bar with appropriate load rating. 8. Follow The American National Standard Institute (ANSI)/The American Society of Mechanical Engineers (ASME) B30.16-1993; Overhead Hoist (Underhung) guide lines for inspections, maintenance, and operations of the overhead hoists. 9. Follow the manufacturer's operating instructions.
