Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that employees
were exposed to being struck or hit by wire coils and/or crane parts due
to "hoist failures":
ROD BREAKDOWN DEPARTMENT:
1.  The Cleveland Tramrail #20 overhead hoist, 2 tons, had the following
deficiencies:
The sheave facing east was not equipped with a guard on the top of the
sheave.
ABATED DURING THE INSPECTION.
2.  The Cleveland Tramrail #19, overhead hoist had the following
deficiency:
The eastern wire rope facing south had one (1) bent section.  ABATED
DURING THE
INSPECTION.
3.  The Cleveland Tramrail #18 had the following deficiencies:
a. The hoist was not marked for capacity. ABATED DURING THE INSPECTION.
b. The hook was shinny and indented.
Among other methods, one feasible and acceptable means to correct the
hazard is to:
1.  Follow the manufacturer's/owner's manual recommendations.
2. Comply with ANSI/ASME B30.16-1993; Overhead Hoist (Underhung)
guidelines for
inspection, maintenance, and operations of the overhead hoists, Chapter
16-1, sections 2.1 - 2.6.
3.  Utilize other engineering methods.
