recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that, employees
were exposed to being struck or hit by loads and/or hoist/crane parts:
POWDER PRODUCTION:
1.  The one-half (1/2) ton capacity Spanco pedestal jib hoist for the #1
furnace had the following
deficiencies:
a.  A 1-ton capacity spreader bar was attached to "scale", which was
unknown weight
and attached to the hook.
b.  Proper means of device(s), stop(s) or equivalent were not utilized to
prevent the
jib/trolley from swinging into or hitting against the duct work for the
ventilation.
2.  Abell - Howe & Company 2-ton capacity pedestal jib crane for the #1
furnace
and dryer had
the following deficiencies:
a.  The hook was worn out on the bottom with indentation.
b.  The safety latch was bent inward and not functioning properly.
