AND PRACTICES TO PROTECT EMPLOYEES FROM CONFINED SPACE HAZARDS
INCLUDING BUT NOT LIMITED TO POTENTIAL ASPHYXIATION, LEAD AND/OR
CARBON MONOXIDE POISONING WHEN THEY ENTER EXPANSION JOINTS.  THOSE
PROCEDURES AND PRACTICES OMITTED INCLUDE BUT ARE NOT LIMITED TO:
1.   THERE WAS NO CONFINED SPACE ENTRY PERMIT.
2.   PERSONNEL WERE NOT EQUIPPED WITH SAFETY HARNESSES, SAFETY
LINES AND/OR EQUIVALENT RETRIEVAL GEAR FOR RESCUE PURPOSES.
3.   A STANDBY PERSON WAS NOT AVAILABLE.
4.   ENTRY INTO THE OCNFINED SPACE WAS NOT PROHIBITED UNTIL INITIAL
TESTING OF THE ATMOSPHERE WAS ACCOMPLISHED; INCLUDING TESTS
FOR OXYGEN, TOXIC SUBSTANCE LEVELS AND FLAMMABLE GASES.
5.   CONFINED SPACE ENTRANCES DID NOT HAVE SIGNS POSTED.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS TO:
1.   FILL OUT AN ENTRY PERMIT PRIOR TO ENTERING THE CONFINED SPACE.
2.   ENSURE EMPLOYEES ENTERING THE EXPANSION JOINT ARE WEARING A
SAFETY HARNESS ATTACHED TO A LIFE LINE AND RETRIEVAL GEAR.
3.   ENSURE THAT EACH PERSON IN THE CONFINED SPACE HAS A STANDBY
PERSON READY.
4.   ENSURE THE ATMOSPHERE OF THE CONFINED SPACE IS TESTED PRIOR
TO ENTRY WITH A CALIBRATED OXYGEN, TOXIC SUBSTANCE AND
FLAMMABLE GAS METER/INDICATOR.
5.   ENSURE A WARNING SIGN IS POSTED INDICATING THE EXPANSION JOINT
AS A CONFINED SPACE.
