employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of exposure to non-ionizing radiation
resulting in
severe burns of the eyes and skin:
a)  Quality Control Dept. - A written standard operating procedure was not
developed
and implemented to ensure that operators and nearby employees were not
exposed to
non-ionizing radiation generated by the Class IV laser and that a laser
safety officer
was not appointed to ensure that all laser operations are performed in
compliance with
the laser safety guidelines of ANSI Z136.1.
Among other methods one feasible and acceptable abatement method to
correct this hazard is
to utilize the directions in the "Guidelines Safety and Hazard
Assessment",
OSHA Pub 8-1.7,
and the National Standard for the safe use of Lasers, ANSI Z136.1,
development of a
Standard Operating Procedure for Class IV laser and the appointment of a
Laser Safety
