Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling load due to failure of wire rope on crane: a) Receiving Department - Employees operated a ROBBINS & MYERS 2 ton hoist with two parts of line on an underhung crane . The 5/16 inch wire rope on the hoist was severely damaged. The rope showed numerous broken wires on the rope from the dead-end connection to the load block when set on the floor. Up to twenty broken wires were noted in one lay of rope with 6 to 8 breaks in one strand in that lay. Kinks were also noted in the rope throughout its length as well as abrasion on the outside wires. ABATEMENT NOTE: Among others one feasible and acceptable abatement method is to replace the wire rope on the crane and to establish an inspection program for all underhung cranes based on the criteria set forth in American National Standards Institute (ANSI) B30.11 Monorail Systems and Underhung Cranes, latest edition.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling load due to failure of the lifting device: a) N.C. Milling Department - A C-clamp was used as a lifting device to connect a the work piece to the hook on the overhead hoist in order to set the piece into a crate. The C-clamp was not an approved lifting device nor was there evidence that the clamp was given a rated load test prior to use. ABATEMENT NOTE: Among others one feasible and acceptable abatement method is to replace the C-clamp with an approved lifting device.
