conveyor belt.
a)  Batch Plant - The employee must stand close to and lean over a
conveyor belt to
dump fiberglass and microsilicas onto the belt and no alarm or visual
device is provided
to alert the employee to the starting conveyor.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to install a warning device.
ABATEMENT NOTE:  The warning device shall be actuated by the controller
device starting
the conveyor and shall continue for a required period of time before the
conveyor starts.  A
flashing light or similar visual warning device may be used in conjunction
with or in place of
the audible device if more effective in particular

employer did not
furnish employment and a place of employment which were free from
recognized
hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a hazard of loads falling from overhead:
a)  Steel Shop - The North American Industry five (5) ton underhung crane
had a
homemade below the hook lifting device that was not tested to ensure
compliance with
American National Standards Institute B30.20a-1987, Below-the-Hook Lifting
Devices.
b)  Steel Shop - The load to be lifted by a North American Industry five
(5) ton
underhung crane was attached to the sling with Covert Snaps, which are not
approved
to overhead lifting.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method
to correct the hazard in instance (a) is to test the below the hook
lifting device, in accordance
with ANSI B30.20a-1987 Section 20-1.4.1.
Covert Snaps are not recommended for overhead lifting by manufacturers
such as Campbell
Chain, which states in its publication for Covert Snaps:  "Warning --
These products should not
be used for overhead lifting or in any application were safety is a
factor."

a)  Yard - Yard Mack, Co. # 23 tractor trailer was used to haul wire
around the yard and
it did not have any brakes or lights, exposing employee(s) to a serious
hazard.
b)  Yard - Mack Water Truck Co. # YTB did not have any windows, the door
on the
drivers side would not stay closed, the parking brake did not hold, none
of the lights
worked, and the drivers seat was broken.
c)  Yard - Ford patch crew truck did not have a operable horn, the seat
was falling apart,
the brake lights and turn signals did not work, and the drivers side door
did not stay
closed.
d)  Yard - GMC Yard Truck did not have any head lights, the left rear tail
light, the left
turn signal and, the windshield wipers did not work, and the windshield
was broken.
e)  Yard - GMC Beam Crew Truck did not have functioning parking lights,
tail lights
and turn signals.
f)  Yard - Blue Toyota did not have functioning right front headlights,
turn signals,
parking brake, tail lights, and parking lights.
g)  Yard - GMC Steel Shop Truck did not have functioning parking and tail
lights, turn
signals, and parking brake, the seat was severely deteriorated, the fender
was missing
on the left rear, the door on the drivers side would not close, and the
floor on the drivers
side was completely rusted through.
h)  Yard - Ford Weld Truck did not have functioning parking and tail
lights and turn
signals.
i)  Yard - Dodge Van Pour Crew did not have functioning turn signals and
the tail lights
and the windshield was cracked.
j)  Yard - Green 4x4 Vault Crew Truck did not have functioning parking and
tail lights,
turn signals, windshield wipers and the horn did not work, and the right
front head light
was missing, and the windshield was broken.
k)  Yard - The new Trojan 1900 rubber tire front end loader, Serial Number
3137105,
has a broken front window and several of the lights do not work, exposing
the
employee(s) to a serious hazard.
l)  Yard - The rubber tire Ford front end loader, Serial Number 6332, has
a
broken
windshield, no back up alarm or horn, numerous lights do not work, the
parking brakedoes not hold, the seat is falling apart, the windshield
wiper does not work, turn signals
do not work and the tires are worn down to the ply, exposing the
employee(s) to a
serious hazard.
m)  Yard - The old Trojan rubber tire front end loader, did not have any
lights, the back
up alarm and horn did not work, the seat had fallen apart, employee(s)
were instructed
to ride on the side of the machine when they were pulling wire through the
form and no
roll over protection was provided, exposing the employee(s) to a serious
hazard.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow recommended vehicle safety precautions and inspections.

a)  Yard - The Drott straddlelift/travelift crane, Serial Number 69A216237
Co. # TL2,
did not have operable travel brakes, no audible alarm, no fenders or
bumpers on the
wheels, the horn does not work, the windows in the door and side are
broken, the seat
is falling apart, no signal chart is on the machine, and the company does
not have an
inspection program.
b)  Yard - The Mi-Jack travelift, Serial Number 6209589, Co. # TL4, did
not have
properly working travel brakes, no inspection program, stress cracks in
the left rear
column, no signal chart, loose bolt in the right column and the windshield
was cracked.
While traveling in the main building, the operators view was obstructed by
the ledger
beam form.  The form is one hundred sixty (160) feet long and eighty six
(86) inches
high.   A spotter was not used, the travel alarm and horn did not work and
the
fender
and bumpers around the wheels were removed, exposing the employee(s) to a
serious
hazard.
