Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing/amputation hazard form unintended operation: a) Sheet Metal Shop - Employee(s) use a Chicago 60 ton mechanical press brake to bend sheet metal (10 foot bed). The foot pedal control was not guarded to prevent accident l actuation by falling or moving objects or by someone accidentally depressing it, on or about 8/24/93. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to construct a barrier guard over the foot pedal. b) Sheet Metal Shop - Employee uses a Chicago 60 ton mechanical press bake to bend sheet metal (6 foot bed). The foot pedal control was not guarded to prevent accidental actuation by falling or moving objects or by someone accidentally depressing it, on or about 8/24/93. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to construct a barrier guard over the foot pedal. c) Sheet Metal Shop - Employee(s) use a 125 ton mechanical Niagara press brake to and sheet metal. the foot pedal control was not guarded to prevent accidental actuation by falling or moving objects or by someone accidentally depressing it, on r about 8/24/93. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to construct a barrier guard over the foot pedal.
