Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing/amputation hazard from unintended activation: a) First Floor - Employee(s) use a Talon equipment, Model 1-10974 wire- stitcher bottom stop. The foot pedal was not guarded by a cover to prevent unintentional activation, on or about August 20, 1993. Among other methods, one feasible and acceptable abatement method to correct this hazard is to place a cover over the pedal to prevent unintended operation from falling or moving objects, or from someone accidentally stepping on the pedal.
