Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer
did not furnish employment and a place of employment which were free from
recognized hazards that were causing or likely to cause death or serious
physical
harm to employees in that employees were exposed to a crushing hazard
from a 2000
pound roll of paper falling eighty two (82) inches because of a severely
worn
hook.
a)  Print department - Six (6) printing presses each have a
electric three
thousand (3000) pound hoist that is used to hoist rolls of paper
weighing
a maximum of two thousand (2000) pounds. The top roll is hoisted
eighty
two (82) inches high and placed in the press rack. The hook
measures 2 1/4
inches at the throat on a new hook and 1 3/4 inches on the worn
hook. This
indicates a twenty two (22) percent hook wear and ANSI requires
only ten
percent (10).

THIS HAZARD IS TO FOLLOW THE MANUFACTURER'S RECOMMENDATION OR THOSE
PUBLISHED
IN THE AMERICAN NATIONAL STANDARDS INSTITUTE B30.10 HOOKS AND NOT ALLOW
THE HOOK
TO WEAR MORE THAN TEN PERCENT (10). Section 10-1.2.3(b)(2).
Employer Knowledge: This item is open and obvious and the company has a
hoist
inspection program and some hooks were replaced.
