were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:

Elevator: Lewis Shepard; Multiply manpower stack, type 13586, capt. 1000,
H.P.-1, Lewis
Shepard Company, Watertown, Massachusetts.  Elevator had not received a
daily safety check,
such as, but not limited to check rollers, roller guides, check control
handle, check stopping
position of unit at both bottom and top landing to ensure proper
functioning of upper and lower
limits.
Among other methods, feasible and acceptable abatement methods to correct
this hazard is to
maintain written, dated and signed inspection reports and records of daily
examination of
elevators in accordance with ANSI A 17.2, 1973, Part I Routine Inspection
and/or comply with
manufacturing recommendation.
