Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a)The hazard of being crushed between backing refuse trucks and objects in
their
path during trash collection operations employing a rear end loader truck
at the
jobsite located at 26010 Southwest Freeway on or about October 23, 2000.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to require truck operators to bring the vehicle to an
immediate stop
whenever helpers on the work crew are not in sight, as required by
paragraph
6.2.3.2 (h) of the American National Standards Institute standard ANSI
Z245.1-
1999, and by the standard operation procedure titled "When Backing": #3

Hazard: Crushed to death by Rear End Loader backing into container.
