Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)an ammonia, compressed gas cylinder on the second floor print shop,
Room 221D
was not properly secured or supported to prevent it from being knocked
over.
This ammonia compressed gas is used in the production of blueprints.  The
hazards of such exposure include being struck by the falling cylinder;
being
struck by the flying cylinders caused by the rocket effect as a result of
the
cylinder valve being broken off when the falling cylinder valve strikes an
object;
employee overexposure to ammonia if the gas is released into the atmosphere
from the compressed gas cylinder; on or about 1/12/01.
ABATEMENT NOTE:
Among others, one feasible and acceptable method to correct this hazard is
to
support the cylinders by using a cylinder truck, chain or steadying device
to keep
cylinder from being knocked over while in storage or use.  Handling,
storage,
and utilization of all compressed gas cylinders, shall be in accordance
with the
Compressed Gas Association Pamphlet P-1-1965, which is incorporated by
reference as specified in 1910.6
Hazard: Struck by/Inhalation.
