Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to possible load failure from a lack of
inspections:
ESTABLISHMENT: In the following instances, workers who are allowed
to operate the overhead hoists located throughout the facility, are
exposed to injury from load failure while operating hoists which
have not had a complete periodic inspection in several years.
These included but are not limited to:
Bay 2  (Torch Bay)
a)Ingersoll Rand 1/2 TON Monorail Hoist SN: Q50-20412.
BAY 4
b)Burlington 1/2 TON Hoist, SN: 490X.
c)Dwight Foote 3 TON Hoist, SN: 798.
Amongst others, one feasible method of abatement would be to set-up
and conduct periodic inspections based on the requirements set
forth in the ASME/ANSI Overhead Hoists (Underhung), Number B30.16.

ESTABLISHMENT: In the following instance, workers who are allowed
to operate hoists are exposed to injury from load failure while
operating the hoists which have not had  frequent (daily to weekly)
visual inspections for any defects or damage that might occur
between periodic inspections, during the several years since the
company has moved to the Bridgeport, Ct. facility.
Bay 4
a) Burlington 1/2 TON Hoist SN: 490X.  The running rope was
observed to have seven (7) broken outer wires in one strand, of one
rope lay.
Amongst others, one feasible method of abatement would be to set-up
and conduct frequent visual inspections based on the requirements
set forth in the ASME/ANSI Overhead Hoists (Underhung), Number
B30.16.
