General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads and/or hoist parts: (a) Facility, Bay 2, Five ton Loadlifter hoist, serial number 21338: The following deficiencies were brought to the attention of the employer and repairs or replacements were not provided promptly to ensure the safe operation of the hoist: (1) Collector wheels need to be replaced. (2) Button covers on the pendant station are worn and/or missing. (3) L3 ground wire needs to be repaired. (4) Hoist motor brake coil and brake disc need to be repaired/replaced. (b) Facility, Bay 2, Ten ton Loadlifter hoist, serial number 27234: The following deficiencies were brought to the attention of the employer and repairs or replacements were not provided promptly to ensure the safe operation of the hoist: (1) Collector wheels need to be replaced. (2) Hoist contactor contact tips need to be replaced. (3) Bridge drive belts need to be replaced. (4) Bridge driver and idler wheels need to be replaced, complete with bearings. (5) Upper limit switch is not functioning properly. (c) Facility, Bay 3, ten ton Loadlifter hoist, serial number 27186: The following deficiencies were brought to the attention of the employer and repairs or replacements were not provided promptly to ensure the safe operation of the hoist: (1) Pendant station needs to be rewired. (2) Hoist single speed button needs to be replaced with a two speed button. (3) Bridge idler wheels and bearings need to be replaced. (4) Upper sheave assembly needs to be replaced. (5) Drive belts on crane need to be replaced. (d) Facility, Blue Room, One ton Whiting Tramrail: The following deficiencies were brought to the attention of the employer and repairs or replacements wee not provided promptly to ensure the safe operation of the hoist: (1) Bridge contactor overloads need to be replaced. (2) Collector tow arm needs to be repaired. (e) Facility, Plantwide: Employees who operate hoists were not trained by a qualified individual on safe operating procedures. ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM
