employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to a crushing hazard:
(a) Facility, Oakmont, PA:  On January 24, 2001 there was no positive
blocking device in place
to prevent employee hands from entering the pinch point area created
between the dump and the
dump pit. The hydraulic pump was broken and the hydraulic lines that
control the dump were
capped off.
Among other methods, one feasible and acceptable method to abate this
hazard is to repair the wheel dump
and prohibit the use of a forktruck to lift the dump when the hydraulic
lines are disconnected.
