employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
At the workplace, located at 11800 Rojas, Suite C-20, El Paso, TX 79336,
On or about
07/30/98 at 09:00: 3 Employees were exposed to struck-by / crushed-by
hazards created
whenever operating a Caterpillar Fork Lift Truck (Model TC 40D, S/N 56131)
without seat belts
/ operator restraint system.
One feasible and acceptable abatement method to correct this hazard would
be:  To have seat
belts / an operator restraint system installed in accordance with
manufacturer's recommendations
and ensure that employees are trained and required to wear them anytime
they operate industrial
forklift trucks.
