Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: At the Jobsite of Pride Well Services, Inc., An employee was exposed to struck-by / crushed-by hazards associated with a backing vehicle when another employee backed a 48,000lb Kenworth (KW) gin pole truck up, to hook onto an Aztec 40' float (trailer), and resulted in the crushing death of one of the employees, as did occur on 11/03/98.,
